This Initial Assessment is for NEW patients of acupuncture. It entails a detailed review of your health and medical history, presenting symptoms and signs, dietary and lifestyle habits, and a physical examination. Based on the use of traditional Chinese medicine diagnostic techniques, a treatment plan tailored to the individual will be proposed. Cherry will take the time to explain the assessment, diagnosis, treatment plan and the informed consent to treatment. Depending on the complexity of individual condition and remaining time available, this initial assessment may or may not include acupuncture treatment. Completing the Acupuncture Intake Form at least a day before this initial appointment will definitely help save more time for your treatment.
