Initial Evaluation
Disappointment is for an initial, new patient, appointment. Please allow 45 minutes for this visit. Online forms will be emailed to you, and must be completed prior to the visit. There is $150 Deposit required, which will be applied to your initial visit cost. This deposit is nonrefundable for cancellations less than four hours and/or no-show appointments. Should you have any questions please call/text the office directly.
