This appointment is for NEW patients. Please complete the new patient intake form online prior to scheduled appointment time and be sure to arrive 20 minutes early to complete your intake and WSIB paperwork or you may not be able to see the chiropractor that day. We require 24 hours notice to cancel or change appointments or you will be charged a cancellation fee. We appreciate your understanding and thank you for your business.
