New Patient Intake (new to North Point chiropractic or have not been seen in the past 3 years). Patient medical (personal and family) history will be taken, physical exam will be performed. Report of finding will be discussed, treatment may be administered if appropriate based on condition(s), treatment plan recommendation will be made.
