Contact before booking. Insurance must be verified by Peak Function Health and prescription acquired prior to beginning care. Prescription/ referral is required. Please include you name, date of birth, insurance carrier name, and subscriber number. Once I have verified your information, you can book your appointment. For your first visit, extra time is reserved so we have the opportunity to discuss your health needs, perform any baseline assessments, and go over paperwork. Please plan for up to 75 minutes.
