New Patient Chiropractic Appointment
Please fill out all the paperwork in advance of the appointment. If you do not complete the paperwork in advance, you will no be able to receive any treatment on the initial visit. You will be asked to put a credit card on file. This is for "no show" appointment fees, or appointments that have not been rescheduled within 24 hours. If you are able to reschedule the same day, you will not be billed a "no show" appointment fee.
