New Patient Initial Assessment
New Patient Initial Assessment 30 Minutes is set aside for your consultation. Your appointment will start at your reserved time. New patient paperwork is sent via email and must be completed 24 hours prior to appointment. Cancellation policy for new patient appointments is 8 hours notice. Cancellation fee of $150 for short notice cancellations or no show.
