Initial Assessment
Thorough examination including history, physical exam, diagnosis, treatment, duration and cost of plan will be discussed. A complete treatment maybe rendered if time permits. Please have/wear sports attire and arrive 10-15 minutes before scheduled time to fill out intake paperwork (if not completed online).
