**Please select this appointment if this is your First Visit for RMT at the clinic** (Initial Assessment & Treatment) During your first visit, we will require that you fill out a confidential health history intake form that can be filled online prior to your appointment. The purpose of reviewing the health history form is to try and create a treatment plan that is best suited for your massage therapy needs. Following the history review, an evaluation will be performed and followed up with massage treatment. If it has been over 1 year since your last massage a review will also be conducted.
