If you are returning but it has been more than 3 months or is a new area of concern, please use the “Initial Existing Physiotherapy Assessment” booking. Note: this booking is not for pelvic health, please see Pelvic Health Initial Assessment. You will be emailed an intake form to the email address provided. Please complete the form before your appointment. All visits are subject to a 24 hour cancellation policy. Missed/rescheduled visits will be charged a cancellation fee in the amount of your visit cost.
