Please book this appointment if it is your first time seeing one of our NDs. You will receive an online intake form to fill out before your appointment. Please bring in all recent blood lab results or diagnostic imaging reports (MRI, ultrasound, X-ray, etc). You can also bring any current medications and supplements. Thank you!
