IMPORTANT! If you are a New Patient or if I haven't seen you in OVER A YEAR, then click this appointment button. We accept Anthem BCBS, Aetna and Cigna. All plans within each insurance company have different benefits. Once you fill out the intake forms we will call to verify your coverage / benefits. We accept Medicare, but we our non participating, which means you will have to pay at the time of service and we will bill out Medicare for you and whatever they pay you receive directly from them. We do not accept Medicaid or Passport. If you have any questions or have another insurance plan not listed above, feel free to reach out to us and we can let you know if there is coverage. We have reasonable cash fees, so if we don't take your insurance there is always that option.
