This 1 hour and 15 minute initial appointment is meant for anyone who is not already a patient of Dr. Fox's or is and existing patient who has not seen her within the last 2 years. We will email you your intake and consent forms 1 week prior to your visit. If you can have these documents ready for Dr. Fox's review 48 hours prior to your appointment we would really appreciate it.
