Be sure to complete your intake form ASAP and allow 48 hours for verification. We currently accept Regence/BCBS of Oregon (60 min), UnitedHealthcare (60 min), Providence Collective Health plans (60 or 90 min), and HSA/FSA cards for Manual Therapy Sessions. Payments and co-pays are due at time of service. Insurance will be billed for services rendered; patient is responsible for any denied claims. Medicare and Medicare supplement plans do not cover these services.
