Please wear comfortable clothing and arrive well hydrated for your appointment. We recommend completing any intake forms before your visit to help ensure a smooth experience.

Initial Intake forms are required to be completed BEFORE the appointment. They take about 30 minutes to be completed. Please don't have coffee or brush your tongue on the day of your assessment. Bring all supplements and medication that you are currently taking to your appointment. Also, the latest labs if they are less than 12 months old. Please note the forms sometimes do not save when completed on a tablet or your mobile device. Please use a computer to complete them.
