By scheduling this appointment you are agreeing to/acknowledging the 12-hour cancelation policy. If an appointment is cancelled within 12 hours of the start time, you will be charged 50% of the total cost. If you fail to show up for your appointment without notifying us, you will be charged the full cost of treatment.
