Comprehensive care. Please fill naturopathic intake forms. Completing the Naturopathic Intake Form before appointments helps your doctor understand your health. If you skip it, you might find yourself diving into your health history and stalling progress on orders or treatments. Save yourself from extra trips just for paperwork! Get it done early and be rewarded with extra time to chat about your current health during your visit. Billed to Insurance or $332.
