***For BCBS PPO insurance billing as a new patient to Prosperian, you must schedule a first office visit with a Prosperian doctor prior to booking with Nick as the insurance billing requires a doctor's prescription for the therapy and medical supervision. These appointments must be scheduled at least 24-hours apart. For non-BCBS PPO insurance patients(self-pay), you may schedule with Nick without a Prosperian doctor's prescription as no insurance claims will be submitted and you will pay out of pocket for services. A new patient intake form will be emailed to you and must be completed at least 24-hours prior to the appointment, or the appointment will be cancelled.
